Reducing risk-factors for Non-Communicable Diseases (NCDs) - Phase II

The Act4Health Phase 2 project builds upon the achievements of Phase 1 to further mitigate risk factors, reduce morbidity and mortality from non-communicable diseases (NCDs), and strengthen Ukraine’s health system resilience. The project continues to address key risk factors such as tobacco and alcohol consumption, physical inactivity, and unhealthy diets while reinforcing public awareness, community engagement, and healthcare capacity.

Given the ongoing challenges, including the impact of the war, Phase 2 will prioritise an enabling policy environment, health system strengthening, and integrated community-based interventions. The project will also expand efforts to ensure continuity of care for NCD patients and improve access to services for vulnerable populations.

Phase 2 of the project focuses on four key outcomes:

  • Strengthening policy frameworks at national and regional levels to effectively address NCDs, ensuring alignment with European and international standards.
  • Expanding comprehensive and innovative approaches for NCD prevention in schools and communities, including workplace health promotion and large-scale public awareness campaigns.
  • Enhancing primary healthcare services to improve NCD prevention, early detection, and management, with a focus on digital innovations and outreach programmes.
  • Improving the quality of life for vulnerable populations through an integrated home-based care model, supporting both medical and social care services.

The project is implemented by a consortium of international and national partners, including GFA Consulting Group, Geneva University Hospitals (HUG), and local organisations. By strengthening partnerships and integrating policy, healthcare, and community-driven solutions, Act4Health Phase 2 aims to ensure long-term sustainability and impact for NCD prevention and care in Ukraine.
